Primary Health Care Services
The Community Health and Behavioral Services division of Upstate Cerebral Palsy, or CHBS, provides primary health care services for children, adolescents and adults on an outpatient basis. Services include routine physicals and exams, treatment for illness, immunizations and health care counseling.
CHBS places an emphasis on preventative health techniques, instructing patients on ways to live healthier lives. Safety lessons, dietary plans and medical advice from our professionals help put patients on the right course to overall wellness.
Specific health care services include: primary health care, endocrinology, podiatry, dental, occupational therapy, speech therapy, and physical therapy. Although services are coordinated at our main office in downtown Utica, satellite offices across the community also offer various health services.
Our talented team consists of physicians, licensed nurse practitioners, physician assistants, registered nurses, licensed practical nurses, as well as knowledgeable staff to assist with billing and Medicaid Services. For more serious health concerns, our professionals will guide patients through the necessary steps to be assisted by our affiliate agencies and/or local hospitals.
Upstate Cerebral Palsy and the Community Health and Behavioral Services division hosts a dental site based in Rome, NY. This office, offering standard dental care, is open to individuals of Upstate Cerebral Palsy as well as community members.
Patients at the Community Health and Behavioral Services can be referred by an outside provider, including the county or state, or can refer themselves, simply by calling or visiting our location.
