Department of Health (DOH) waiver services are offered for individuals who have suffered Traumatic Brain Injury. Working in partnership with the person or their family, MSC will work to explore the individual’s strengths, capacities and goals for the future. Once a plan is in place, the service coordinator will help to secure the supports and services necessary to meet these goals. These services are provided by Upstate Cerebral Palsy in Oneida, Lewis, Herkimer and Madison and Otsego Counties.
Independent Living Skills Training supports individuals to improve and maintain community living skills so that the individual can live as independently as possible. Training is provided one on one in the individual’s home and the community and focuses on needs such as shopping, cooking, money management, use of public transportation, etc.
Home and Community Support Services are personal aide services that compliment medically based personal aide services. Includes supervision, companion and chore services, which can be provided in the individual’s residence or in the community.
Environmental Modifications are physical adaptations to the individual’s residence and primary vehicle which ensure the individual’s health, safety and welfare, and which increases the individual’s independence and integration in the community.
Who is eligible?
To be eligible for this Medicaid Waiver, an individual must be:
• Diagnosed with TBI or a related condition
• eligible for nursing facility level of care
• enrolled in the Medicaid program
• 18-64 years old
